Academic Complaints Form

Please use this form to convey any complaints or concerns you have regarding an academic matter in a course offered by the Department of Microbiology & Immunology (MIMM). 

Submissions will be addressed within one business week, except disrespectful ones which will not be considered. While identifying yourself is optional, please note that our ability to address your concern may be limited without this information. If you do identify yourself, your name and contact information will remain confidential and will not be passed along without your consent. We may reach out to request additional information and/or follow-up with updates. 

Please note that submission of a complaint does not guarantee that the solution you desire will be reached, but I will do my best to ensure your voice is heard by the Department.
